Overview of FMT Costs for International Patients
Fecal microbiota transplantation (FMT) is provided by medical institutions affiliated with the Fecal Microbiota Transplantation Clinical Study Group in Japan, in collaboration with Shinbiosis Corporation.
FMT is performed as free medical care, meaning it is not covered by public health insurance in Japan.
FMT is conducted using a standardized procedure in which bacterial solutions are prepared with hydrogen nanobubble water for all patients.
For international patients, this standardized procedure is provided as part of coordinated care in collaboration with affiliated medical institutions in Japan.
The number of transplant sessions, treatment plan, and overall cost vary depending on the patient’s medical condition, disease history, and clinical judgment by the physician.
Because FMT may be performed over multiple sessions, the total cost differs from patient to patient.
After an initial consultation and review of medical information, a personalized transplant plan and cost estimate will be proposed.

What Determines the Total Cost of FMT
The total cost of fecal microbiota transplantation (FMT) is determined by several factors related to the patient’s condition and treatment plan.
One key factor is the number of transplant sessions required.
Depending on the disease, severity of symptoms, and clinical response, FMT may be performed as a short course or as multiple sessions over time.
The patient’s medical condition and treatment objectives also influence the overall cost.
Based on medical information provided in advance and the physician’s assessment, an appropriate transplant plan is designed for each individual patient.
In addition, the total cost includes pre-transplant support and coordination, which is provided to international patients to ensure smooth communication and preparation before the first visit to Japan.
Please note that medical fees charged by affiliated medical institutions are set independently.
These fees are paid directly to the medical institution on the day of consultation or treatment and are not included in the support fees managed separately.
Pre-transplant Support Fee
For international patients, a pre-transplant support fee is charged in addition to medical fees.
This support is designed to assist patients before their first in-person consultation with a physician in Japan and to ensure smooth communication throughout the preparation process.
The pre-transplant support fee is charged per treatment course, regardless of whether a 6-transplant course or a 3-transplant course is selected.
What Is Included
The pre-transplant support fee include the following services:
- One online meeting with a dedicated concierge
- E-mail inquiries to the concierge until the patient has a face-to-face consultation with a physician
- Two online consultations with a physician
- One face-to-face consultation with a physician
- One flora balance test
- Interpretation services
(including one meeting and up to three consultation sessions)
These services are provided to support communication, coordination, and preparation prior to the start of FMT treatment.
What Is Not Included
The following items are not included in the pre-transplant support fee:
- Medical examinations, tests, treatments, or physical examinations other than the flora balance test
- Nutritional guidance or additional counseling services
- Travel, transportation, accommodation, or sightseeing expenses
- Daily living expenses during the stay in Japan
- Additional interpretation services beyond the included scope, if required
If required, travel-related arrangements may be provided as a separate paid option.
Medical fees prescribed by each affiliated medical institution are charged separately and must be paid directly to the medical institution on the day of consultation or treatment.

Additional Transplants
In some cases, additional fecal microbiota transplantation (FMT) sessions may be recommended based on the patient’s condition and clinical response.
Additional Fees After the Standard Course
For patients who have completed a standard treatment course, additional transplant sessions are available under the following conditions.
- After the 7th transplant of the 6-transplant course
Additional FMT session: 660,000 yen (Tax included) - For the 4th to 6th transplants of the 3-transplant course
Additional FMT session: 715,000 yen (Tax included) - After the 7th transplant of the 3-transplant course
Additional FMT session: 660,000 yen (Tax included)
The need for additional transplant sessions is determined by the physician based on medical evaluation and treatment progress.

Payment methods may vary depending on the medical institution.
Please note that fees related to pre-transplant support and coordination are managed separately from medical fees.
If you decide to cancel a meeting, consultation, or treatment during the course of the program, fees will be refunded in accordance with the applicable cancellation policy.
Details of the applicable cancellation policy will be provided to each patient individually and explained as part of the consent process prior to proceeding with treatment.
